OUR APPROACH TO CORE SUPPORT WITH COMPLEX CARE NEEDS

Relyability Care delivers core support through a comprehensive framework of clinical and mental health oversight, ensuring participants receive safe, responsive, and person-centred care.

Our approach is supported by registered nurse oversight, medication risk management, clinical input into support planning, and the implementation of behaviour support strategies. We also provide mental health-informed supports, clear escalation pathways, and staff guidance for participants with complex behavioural or psychosocial needs.

In addition, we can also assist with hospital discharge and step-down supports, helping individuals transition safely while maintaining continuity of care and support.

For participants who require core support, we can provide these specialised services as part of their existing support arrangements. We work closely with each individual to ensure their complex care needs are met with the appropriate expertise, oversight, and ongoing support. This means participants can receive consistent and personalised care.

SUPPORTED INDEPENDENT LIVING (SIL)

Supported Independent Living is an NDIS support designed for people who need assistance with daily tasks and developing greater independence.

SIL is designed around the individual, which means the type and level of support should reflect a participant’s needs, preferences and goals.

PARTICIPATION IN THE COMMUNITY

Community participation is about being involved in activities, places and social environments that are meaningful to an individual.

For one person, this could mean joining a local sports club. For another, it might involve attending an art class, visiting a library, taking part in a community event or developing the confidence to meet new people.

WHY THE RIGHT SUPPORT DOESN’T RUSH PROGRESS

When people think about progress, they often picture moving faster, achieving goals quickly, or reaching major milestones. However, for many NDIS participants, progress is not measured by speed. It is measured by confidence, comfort, and the ability to move forward in a way that feels meaningful and sustainable.

This is where person-centred disability support makes a difference. Rather than following a one-size-fits-all approach, quality disability support focuses on the individual, recognising that everyone experiences growth differently.

UNDERSTANDING NDIS ELIGIBILITY REQUIREMENTS

The National Disability Insurance Scheme (NDIS) was designed to support Australians with significant and permanent disability — but it’s not intended to cover every disability need. Other government systems such as health, education, housing, employment services, and social security also play a role.
